Organizations should work with their VISTA candidate(s) to submit all of the following information together.

Organization Specific Paperwork
- Signed Form 424 Facesheet This is page 1 of the PDF generated by the CTC VISTA Project's online organizational application system.
- Signed Assurances and Certification forms: To be signed by the ED or board president.
- Two signed copies of the Memo of Understanding. This is the agreement between the host organization and the CTC VISTA Project. A board signature is required on the last page.
- Financial Obligations: This clearly outlines the financial obligations the host organization will assume upon placement of a VISTA member and should be signed by either the Executive Director or Board Member of the host organization.
- Supervisor’s Agreement: This should be signed by the full-time staff member who will be working most directly or on a day-to-day basis with the VISTA member.
- Sponsor Evaluation. This is the your organization's evaluation of how suitable your VISTA candidate is for the position described your organizational application.
VISTA Specific Paperwork
- Official AmeriCorps*VISTA Application: Must be completed online and the application submitted electronically (through the official system) to the "Community Technology VISTA Project". From the link, click the teensy-tiny red link for "Apply to Serve" below the login-box. This isn't our website, but please let us know if it's not working
- Two Completed References: This is part of the Official AmeriCorps*VISTA Application, but references must be completed for the candidate to be considered. If the references are notcompleted online, the host organization can personally interview the references using our recommendation form [Word].
- Proof of Citizenship: A list of appropriate document(s) that must be submitted to provide evidence of citizenship, national or permanent residency of the VISTA candidate.
- Signed VISTA Assignment Description:This is part of the PDF generated by the CTC VISTA Project's online organizational application system (the same PDF that the 424 form is in). This description serves as the initial working agreement between your organization and your VISTA. It should be signed by both your VISTA candidate and their supervisor.
- Travel Form: Should be completed in regards to how the VISTA candidate will be traveling from their current home, to our Pre-Service Orientation in Boston, and then to their host site. This is used by us to purchase airplane tickets for the VISTA candidate. The Pre-Service Orientation (PSO) will take place July 8 - 11 in Boston, MA. All airfare, hotel and food costs are paid by the CTC VISTA Project.. VISTA members will begin service immediately following the PSO.
